Use this service if you have to stay at home because of coronavirus and you need a note for your employer.
If you have to stay at home but feel well enough to work, ask your employer if you can work from home. If you can work from home, you will not need an isolation note.

You can mark yourself as having arrived for your appointment yourself by using the screen in reception; the screen will tell you which waiting room to go to.
The Holborn Medical Centre offers a variety of appointments;
To help prevent wasted appointments, you will receive a reminder text message from the practice 48 hours in advance of your appointment. If you wish to cancel your appointment you will be able to do so via text message.
Home VisitsIf possible please try to telephone reception before 10am if you require a home visit.
A doctor or nurse may phone you back as it may be that your problem can be dealt with by telephone advice, or that it would be more appropriate to send a nurse, or indeed arrange a hospital attendance.
House visits are only available for patients who are housebound because of illness or disability.
Please remember that several patients can be seen in the practice in the time that it takes to make one home visit. There are also better facilities for examining and treating patients at the Health Centre.
Results of various tests can take up to five working days and usually arrive in the afternoon.
Please phone for your results, or call in, before 1.00pm.
